When Mongoose published Traveller, that meant that everyone else's licenses sunsetted (i.e. couldn't be renewed after they expired).
Whether that idea came from Mongoose or Marc, I don't know - I think the idea was to "unify" Traveller under the Mongoose Traveller/OGL,m but then Marc went and totally undermined that by working on T5, which is rather different to MGT.
Avenger were, IIRC, using Mongoose's Flaming Cobra imprint as a way to get their CT and 1248 products printed and distributed to gamestores.
As it stands, the only people who can now publish OTU material are Mongoose, SJG (though they haven't done anything new for years - I think they're just using their license to be able to continue to sell GT books), and FFE.
